Get Copies of Passport Records

We have passport records from March 1925 to the present. The Privacy Act of 1974 protects records. Records may include:

Passport records do not include evidence of travel such as entrance or exit stamps, visas, or residence permits.

You can get:

  • Copies of your records
  • Your minor child's records
  • The records of any person for whom you are acting as a legal guardian
  • The records of any person who has authorized you to get them

Law enforcement officials can request records of someone they are prosecuting or investigating.

How to get Passport Records

Send a written request for records to:

The written request must include:

  1. Full name at birth and any names used for the individual whose records you are requesting
  2. If you are requesting records for someone else:
    • Provide your full name 
    • Show you have legal authority to request records, and include evidence of your relationship
    • Use DS-4240 to provide proof of authorization
  3. Date and place of birth of the individual whose records you are requesting
  4. Your contact information including mailing address, phone number, and email address
  5. Passport information including the date or estimated date issued and the number (if known)
  6. Any other information that will help us locate the records
  7. A clear copy of both sides of your valid, government-issued photo ID such as a driver's license
  8. Your signature in front of a notary public, or your signature on a 'penalty of perjury' statement

Processing Times

  • 12 to 16 weeks

Fees

  • There are no fees to request a regular copy of passport records
  • If you want certified copies of these records, the certification fee is $50
  • Make checks or money orders payable to “U.S. Department of State.” 

Freedom of Information Act (FOIA) Requests

Submit a request if you want someone's passport records, and:

  • You do not have an authorization from that person, or 
  • The person is deceased
