Passport Surveys and Renewal Reminders
Your answers to our surveys help us improve our services. You may receive a letter or email inviting you to complete a survey or reminding you to renew your passport.

We send letters every month to some customers inviting them to complete a Passport Demand survey. Your response helps us estimate demand for passports and manage our workload. Your feedback also helps improve our customer service and develop new products. You can reply to the survey online or over the phone.
If you have questions, email us at passportstudy@state.gov. For more information, please visit the Passport Demand Study's official Federal Register page.

You can also review a sample version of the letter, which will always be on official Department of State letterhead. The phone number in the letter you receive may be different than the one in this sample version.

We send emails to some recent customers to get their input on the passport application experience. Some questions will ask you about the trust you have in the U.S. Department of State. The email sender will be noreply@qemailserver.com. The subject line of these emails will be "Your Feedback Requested - U.S. Passport Application."
If you have questions about this survey, email us at consularcustomerresearch@state.gov.
