About the commemorative passport
The Department of State created a special U.S. passport ("Patriot Passport") to commemorate the historic occasion of America’s 250th anniversary. This special passport has custom artwork and enhanced images on the front, back, and inside covers. It has all the same advanced security features that make the U.S. passport the most secure travel and identity document in the world. The Department is now offering Americans across the country the opportunity to apply for one of these 250,000 special and historic passports.

Issuance of the commemorative passport
In response to popular demand, the Department of State is offering Americans across the United States special opportunities to apply for the limited-edition commemorative passport ("Patriot Passport") at all of our passport agencies and centers. We still continue to issue the special passport at the Washington Passport Agency in Washington, D.C. Applicants seeking the commemorative passport may should call 1-877-487-2778 to schedule an in-person appointment at the Washington Passport Agency or at a special passport acceptance event listed on this page.
You will not receive a "Patriot Passport" passport if you:
- Apply online
- Mail in your application
- Apply at an acceptance facility
- Apply at a U.S. embassy or consulate abroad
Option 1: Make an in-person appointment at the Washington Passport Agency in Washington, D.C.
- Dial 1-877-487-2778 and select option 9 to check appointment availability at the Washington Passport Agency. Appointments cannot be made online.
- Once your appointment is booked, you will receive an appointment confirmation email.
- If you do not have imminent international travel, your passport will be mailed to you.
Option 2: Make an in-person appointment at a 250th Anniversary commemorative passport application event at a passport agency or center near you.
- Dial 1-877-487-2778 and select menu option 9 to check appointment availability and schedule an appointment. Appointments cannot be made online.
- Once your appointment is booked, you will receive an appointment confirmation email.
- Regular fees and processing times will generally apply, and your commemorative passport will be mailed to you.

The commemorative passport has 26 pages, just like the standard passport book design. Large format passport books have 52 pages, but use the existing design, not the commemorative passport artwork.
If you make an appointment at the Washington Passport Agency in Washington, D.C. and request a large passport book, it cannot be printed onsite. We will collect your materials and electronically transfer your application to a facility where we print large passport books. Your new passport can be sent to you via standard USPS Priority mail or trackable 1 to 3 day delivery.

Applications for commemorative passport use the State Department's established passport application forms, the DS-82 or DS-11, depending on your eligibility.
