Know Before You Go: Travel Advisories for Out-of-State Customers

  • In response to increased rates of COVID-19 transmission, many states have travel advisories for anyone traveling from states that have a significant degree of community-wide spread of COVID-19.
  • The advisories may require out-of-state travelers to self-quarantine for 14 days. Exceptions may apply for transiting through the state by car, train, bus, or airplane.
  • Customers who want to schedule an appointment at an agency or center that is located out of state should check for travel advisories in the state where the passport agency or center is located.

1. Find out if you're eligible

To apply at this passport agency, you must have a life-or-death emergency which requires you to travel internationally in the next 72 hours (3 business days). 2. Collect all your application materials

 
  • Appointment confirmation number
  • Proof of the life-or-death emergency such as a death certificate, a statement from a mortuary, or a signed letter from a hospital or medical professional. Documents must be in English or translated in English.
  • Printed proof of international travel
    • If you are flying internationally, you must provide a flight receipt or an itinerary. If you are traveling by land or sea to Canada, Mexico, Bermuda, or the Caribbean, you can provide a hotel reservation, cruise tickets, or international car insurance.
  • Completed form(s)*
  • All required supporting documents*
  • One passport photo
  • Application fee and expedite fee. We accept:
    • Major credit cards
    • Debit/Check card (not ATM cards)
    • Cash (Exact amount only- no change provided)
    • Checks
    • Bank Draft or Money order
  • If you are applying for a child under 16, both parents and child must be present to apply. You must submit documentation that shows parental relationship to that of the child applying for a passport.  See Children Under 16 for a full list of requirements and documents.

3. Make an appointment

You must make an appointment to apply at a passport agency. Be sure to write down your appointment confirmation number.

4. Arrive early to your appointment

Customers are required to wear face coverings to be able to enter the agency.
  1. We suggest you arrive 15 minutes early to go through security screening. Weapons, food, and drinks (except water in a sealable bottle) are not allowed.Please note: you may not enter the building before 8:30 am.
  2. Check in at the Information Desk. Give them your appointment confirmation number to verify your appointment.
  3. Be seated and wait until we call your number.
  4. When we call your number, go to your assigned window.
  5. A Department of State employee will review your application and supporting materials and discuss available delivery options.

Parking Information

Public parking is available in the parking lot on the right-hand side of the building located at 7333 East Rosewood St or in the parking lot across the street.
