COVID-19 Update: Safely Working for You

August 3, 2020

To prevent the spread of COVID-19 and protect our workforce and customers, we significantly reduced passport operations in March 2020. We temporarily suspended expedited passport processing and restricted service to cases involving life-or-death emergencies. As global conditions evolve and U.S. states begin to reopen, we are resuming operations in phases. We ask for your patience as we safely work on your application.

As of Monday, August 3, a total of 6 passport agencies and centers are in phase two, and 12 agencies and centers are in phase one of our three-phase reopening plan. Go to our Passport Agency and Center page for more information.

You can apply at an acceptance facility or renew by mail now, but unless you have a life-or-death emergency, you will experience delays before receiving your passport and the return of your citizenship documents such as previous passports, and birth and naturalization certificates.

How to Apply at a Passport Agency

1. Find out if you're eligible

If you have a life-or-death emergency, please see Life-or-Death Emergencies.

2. Collect all your application materials

We have an online form filler so you can fill out your form and print it before you arrive for your appointment. You can also print a PDF of your form and complete your form by hand in blue or black ink before you arrive. Both these options reduce the amount of time you spend in our lobby on the day of your appointment. While we have forms available in our lobby, we ask that you bring your own blue or black ink pen to protect the health of other customers and our employees.
  • Appointment confirmation number
  • Printed proof of international travel
    • Examples: Flight receipt, flight itinerary, hotel reservation, cruise tickets, international car insurance
  • Completed form(s)*
  • All required supporting documents*
  • One passport photo
  • Application fee and expedite fee. We accept:
    • Major credit cards
    • Debit/Check card (not ATM cards)
    • Cash (exact amount only- no change provided)
    • Checks
    • Bank Draft or Money order
  • If you are applying for a child under 16, both parents and child must be present to apply. You must submit documentation that shows parental relationship to that of the child applying for a passport.  See Children Under 16 for a full list of requirements and documents.

3. Make an appointment

You must make an appointment to apply at a passport agency. Be sure to write down your appointment confirmation number.

4. Arrive early to your appointment

All customers entering the Miami Passport Agency must wear a facial covering in accordance with Miami Dade County Emergency Order 20-20. A facial covering includes any covering which snugly covers the face and mouth whether store bought or homemade, and which is secured with ties or ear loops.

  1. We suggest you arrive 30 minutes early to go through security screening. Weapons, food, and drinks (except water in a sealable bottle) are not allowed.
  2. Report to the first floor of the Omni Building’s NE 15th Street entrance and wait to be called for your appointment.
  3. When prompted, take the elevator to the 3rd floor.
  4. Check in at the Information Desk. Give them your appointment confirmation number to verify your appointment.
  5. Be seated and wait until we call your number.
  6. When we call your number, go to your assigned window.
  7. A Department of State employee will review your application and supporting materials and discuss available delivery options.

Parking Information

Metered parking is available on Biscayne Blvd and North Bayshore Drive. There is also a parking garage (hourly and daily fees apply) in the OMNI building, as well as adjacent parking lots nearby. Parking rates subject to change without notice.

Travel Advisory for States

  • In response to increased rates of COVID-19 transmission, several states and cities have placed travel advisories for anyone traveling from states that have a significant degree of community-wide spread of COVID-19.
  • The travel advisories may require travelers to self-quarantine for 14 days. Additionally, exceptions may apply for transiting through the state by car, train, bus, or airplane.
  • Customers who schedule an appointment at an agency/center that is located out of state should check for travel advisories of the state where the passport agency/center is located.
