Each agency or person listed below has been subject to one of the following actions:

• It applied for accreditation or approval and was denied because the accrediting entity determined that the agency or person had not demonstrated substantial compliance with applicable standards in 22 CFR Part 96;
• It has had its accreditation or approval suspended, cancelled, or not renewed by the accrediting entity because the accrediting entity determined that the agency or person had not demonstrated substantial compliance with applicable standards in 22 CFR Part 96, or because the Department of State determined that the agency or person was substantially out of compliance with applicable standards in 22 CFR Part 96; or
• It was temporarily or permanently debarred from accreditation or approval because the Department of State determined that there was substantial evidence of noncompliance with applicable standards in 22 CFR Part 96, and that there was a pattern of serious, willful, or grossly negligent failures to comply on the part of the agency, or other aggravating circumstances such that its continued accreditation or approval was not in the best interests of the children and families concerned
