How to Add Extra Pages to Your U.S. Passport

You have less than 2 to 4 Visa pages remaining in your passport book

 You must mail:

check box Applicable fees  and

check box  Your current, valid U.S. Passport and

check box Completed Form DS-4085, Application for Additional Visa Pages

to the address listed on the Form DS-4085.

THEN

You travel extensively and are applying for a new passport book

You must submit:

check box Applicable fees  and

check box A signed, written request for additional visa pages and

check box If eligible, mail Form DS-82 to the address listed on the form (if not eligible, follow instructions for submitting Form DS-11 in person).
