Incoming Hague Application Checklist


Note: Be certain to check with the officer for your country for any additional or specific requirements.

check  Application form – one signed original for EACH child. NOTE: Some countries may require use of a special application form.

check  Marriage Certificate (if applicable). May need to be certified copy.

check  Birth Certificate for EACH child. May need to be certified copy.

check  Divorce Decree (if applicable). May need to be certified copy.

check  Evidence of Custodial Right

Example:

  1. Custody order, or
  2. Copy of state or foreign law sections relevant to custody, or
  3. Affidavit of law regarding presumption of custody order under state or foreign law.

check    Other relevant court documents

check    Photographs of taking parent and child

check    Statement regarding the circumstances of removal or detention

check    Possible address(es) and telephone numbers for the child in the United States

check    Translations of all documents not provided in English

check    Indication if you would like to pursue a voluntary return

check    Application for legal assistance (if applicable)
